doing a detox in this fashion successfully, requires a doctor who is willing to let you have take home doses, and preferably an understanding pharmacist who you can speak to everyday when you go to pick up the take home doses. it also requires that your doctor and pharmacist communicate, because the doctor isnt going to see you everyday, and needs to know how you are shaping up when you go to collect your doses.
started off on 12MG subutex
/day, and had to do 2MG in the chemist supervised. knowing you only have a limited amount per day helps to stretch yourself between doses.
the next day she only needed ~6-8MG, again she had to dose 2MG of that supervised at the pharmacy.
the following day she only needed ~3-4MG, again 2MG was supervised at the pharmacy.
from then on her doctor gave her temgesic
, and the doses for the last couple days were 0.8MG, 0.2MG- these doses didnt require supervised dose. swims doctor had prescribed her 2MG of temgesic tablets (0.2MG tablets).
then the next day she went to do a narcan challenge to flush the rest of the bupe out of her system. her doctor had worked it out that it might have been around 10MG, if that. instead she opted for low dose naltrexone, which was given in 2MG doses every couple of hours- this was done in a small regional hospital. she was there for around 24 hours.
shes been clean ever since.
swims doctor was a champion. he gave her his after hours pager number, and requested that she made contact everyday, not only to talk about any withdrawal
issues, but to just chat
if need be. he would ring her back ASAP. he also did the naltrexone dosing in hospital (got it covered by medicare and all so it didnt cost a thing- after all, its cheaper for the government to help with recovery in the long run), and dosed her up with chlonodine, promethazine and temazepam
, just in case it precipitated a withdrawal. however swim slept all day, had no withdrawals, and the next day she took a whole naltrexone tablet, had no withdrawals, went home and has been clean ever since.
depending on how much higher swiys methadone
dose is, is dependent, but swim was averaging 100MG of IV methadone when she detoxed, and was fine. that particular detox was the easiest swim ever attempted. maybe it was the frame of mind, and the lovely, supportful doctor, and the pharmacy workers were all lovely too. all very encouraging.
it wasnt the normal pharmacy that swim went to. she wanted to get away from the usual customers, as she had developed quite a dislike for them.
it also helped in the sense that swim approached the pharmacy, and told them of her plans to detox/make a better life, and asked if they would mind dosing her during this time. temporarily. seemed to make them understand, and want to help/want to see swim be successful.
dunno if any of that will help you, cause swims opiate
specialist is an absolute champion, and id be surprised if any other doctor came close to doing the right thing/helping/supporting in that fashion.
swim also returned all the unused subutex to the pharmacist every morning, to avoid having a lot about. oh and one more thing, dont inject it. the first day of swims detox she injected it, and then spent the rest of the day feeling like absolute fucking shit.
